Inquiring into activist publics in chronic environmental issues: Use of the mutual-gains approach for breaking a deadlock
© 2015 John Wiley and Sons, Ltd. This study aims to understand different publics' communicative behaviors for problem solving surrounding an oil spill issue in Korea. Specifically, it explores the differences between chronic activists and other types of publics who were affected by this chronic environmental issue. A total of 24 interviews were conducted, from which five different types of publics were identified. The findings suggest that the majority of activists who are currently working on the issue are closed-chronic activists, which are slightly different from Ni and Kim's findings on chronic activists' communicative behaviors. Interviews were also conducted with five communication experts to propose viable conflict resolution strategies for the issue. The mutual-gains approach is recommended as a viable organization-public conflict resolution strategy
Intestinal epithelial cells: at the interface of the microbiota and mucosal immunity.
The intestinal epithelium forms a barrier between the microbiota and the rest of the body. In addition, beyond acting as a physical barrier, the function of intestinal epithelial cells (IECs) in sensing and responding to microbial signals is increasingly appreciated and likely has numerous implications for the vast network of immune cells within and below the intestinal epithelium. IECs also respond to factors produced by immune cells, and these can regulate IEC barrier function, proliferation and differentiation, as well as influence the composition of the microbiota. The mechanisms involved in IEC-microbe-immune interactions, however, are not fully characterized. In this review, we explore the ability of IECs to direct intestinal homeostasis by orchestrating communication between intestinal microbes and mucosal innate and adaptive immune cells during physiological and inflammatory conditions. We focus primarily on the most recent findings and call attention to the numerous remaining unknowns regarding the complex crosstalk between IECs, the microbiota and intestinal immune cells

The impact of diabetes on multiple avoidable admissions: a cross-sectional study
Background
Multiple admissions for ambulatory care sensitive conditions (ACSC) are responsible for an important proportion of health care expenditures. Diabetes is one of the conditions consensually classified as an ACSC being considered a major public health concern. The aim of this study was to analyse the impact of diabetes on the occurrence of multiple admissions for ACSC.
Methods
We analysed inpatient data of all public Portuguese NHS hospitals from 2013 to 2015 on multiple admissions for ACSC among adults aged 18 or older. Multiple ACSC users were identified if they had two or more admissions for any ACSC during the period of analysis. Two logistic regression models were computed. A baseline model where a logistic regression was performed to assess the association between multiple admissions and the presence of diabetes, adjusting for age and sex. A full model to test if diabetes had no constant association with multiple admissions by any ACSC across age groups.
Results
Among 301,334 ACSC admissions, 144,209 (47.9%) were classified as multiple admissions and from those, 59,436 had diabetes diagnosis, which corresponded to 23,692 patients. Patients with diabetes were 1.49 times (p < 0,001) more likely to be admitted multiple times for any ACSC than patients without diabetes. Younger adults with diabetes (18–39 years old) were more likely to become multiple users.
Conclusion
Diabetes increases the risk of multiple admissions for ACSC, especially in younger adults. Diabetes presence is associated with a higher resource utilization, which highlights the need for the implementation of adequate management of chronic diseases policies.NOVASaudeinfo:eu-repo/semantics/publishedVersio

Observation of the thermal Casimir force
Quantum theory predicts the existence of the Casimir force between
macroscopic bodies, due to the zero-point energy of electromagnetic field modes
around them. This quantum fluctuation-induced force has been experimentally
observed for metallic and semiconducting bodies, although the measurements to
date have been unable to clearly settle the question of the correct
low-frequency form of the dielectric constant dispersion (the Drude model or
the plasma model) to be used for calculating the Casimir forces. At finite
temperature a thermal Casimir force, due to thermal, rather than quantum,
fluctuations of the electromagnetic field, has been theoretically predicted
long ago. Here we report the experimental observation of the thermal Casimir
force between two gold plates. We measured the attractive force between a flat
and a spherical plate for separations between 0.7 m and 7 m. An
electrostatic force caused by potential patches on the plates' surfaces is
included in the analysis. The experimental results are in excellent agreement
(reduced of 1.04) with the Casimir force calculated using the Drude
model, including the T=300 K thermal force, which dominates over the quantum
fluctuation-induced force at separations greater than 3 m. The plasma
model result is excluded in the measured separation range.Comment: 6 page
Combination therapy of disseminated coccidioidomycosis with caspofungin and fluconazole
BACKGROUND: The current recommended therapy for diffuse coccidioidal pneumonia involves initial treatment with amphotericin B deoxycholate or high-dose fluconazole, followed by an azole after clinical improvement. Amphotericin B is more frequently used as initial therapy if the patient's deterioration is rapid. CASE PRESENTATION: A 31-year-old Korean male with coccidioidomycosis presented to the hospital with miliary infiltrates on chest X-ray (CXR) and skin rash on the face and trunk. Initially, the patient did not respond to amphotericin B deoxycholate therapy. However, following caspofungin and fluconazole combination therapy, the patient showed favourable radiological, serological, and clinical response. CONCLUSION: This appears to be the first case of diffuse coccidioidal pneumonia with skin involvement in an immunocompetent patient who was treated successfully with caspofungin and fluconazole. Combination therapy with caspofungin and fluconazole may, therefore, be an alternative treatment for diffuse coccidioidal pneumonia that does not respond to amphotericin B deoxycholate therapy
Strain-relief by single dislocation loops in calcite crystals grown on self-assembled monolayers
Most of our knowledge of dislocation-mediated stress relaxation during epitaxial crystal growth comes from the study of inorganic heterostructures. Here we use Bragg coherent diffraction imaging to investigate a contrasting system, the epitaxial growth of calcite (CaCO3) crystals on organic self-assembled monolayers, where these are widely used as a model for biomineralization processes. The calcite crystals are imaged to simultaneously visualize the crystal morphology and internal strain fields. Our data reveal that each crystal possesses a single dislocation loop that occupies a common position in every crystal. The loops exhibit entirely different geometries to misfit dislocations generated in conventional epitaxial thin films and are suggested to form in response to the stress field, arising from interfacial defects and the nanoscale roughness of the substrate. This work provides unique insight into how self-assembled monolayers control the growth of inorganic crystals and demonstrates important differences as compared with inorganic substrates
Speed and Accuracy of Static Image Discrimination by Rats
When discriminating dynamic noisy sensory signals, human and primate subjects
achieve higher accuracy when they take more time to decide, an effect
attributed to accumulation of evidence over time to overcome neural noise. We
measured the speed and accuracy of twelve freely behaving rats discriminating
static, high contrast photographs of real-world objects for water reward in a
self-paced task. Response latency was longer in correct trials compared to
error trials. Discrimination accuracy increased with response latency over the
range of 500-1200ms. We used morphs between previously learned images to vary
the image similarity parametrically, and thereby modulate task difficulty from
ceiling to chance. Over this range we find that rats take more time before
responding in trials with more similar stimuli. We conclude that rats'
perceptual decisions improve with time even in the absence of temporal
information in the stimulus, and that rats modulate speed in response to
discrimination difficulty to balance speed and accuracy
Casimir forces on a silicon micromechanical chip
Quantum fluctuations give rise to van der Waals and Casimir forces that
dominate the interaction between electrically neutral objects at sub-micron
separations. Under the trend of miniaturization, such quantum electrodynamical
effects are expected to play an important role in micro- and nano-mechanical
devices. Nevertheless, utilization of Casimir forces on the chip level remains
a major challenge because all experiments so far require an external object to
be manually positioned close to the mechanical element. Here, by integrating a
force-sensing micromechanical beam and an electrostatic actuator on a single
chip, we demonstrate the Casimir effect between two micromachined silicon
components on the same substrate. A high degree of parallelism between the two
near-planar interacting surfaces can be achieved because they are defined in a
single lithographic step. Apart from providing a compact platform for Casimir
force measurements, this scheme also opens the possibility of tailoring the
Casimir force using lithographically defined components of non-conventional
shapes
